When NO is locally released from sexual stimulation, sildenafil inhibits PDE5 and increases the amount of cGMP in the corpus cavernosum, which relaxes smooth muscles and increases blood flow into the corpus cavernosum, producing an erection. Recommended dose of sildenafil has no effect in the absence of sexual stimulation. PDE5 is present in other tissues including platelets, abdominal smooth muscle, skeletal muscle, which may be the basis for inhibition of platelet thrombogenesis and peripheral arteriovenous dilatation.
